Athlete's Information

TRYOUT PARTICIPATION & WAIVER RELEASE

Introduction

By registering my child for BTG Basketball tryouts, I confirm that I am the parent or legal guardian of the athlete and that I am providing consent for my child to participate in all tryout-related activities organized by BTG Basketball.

I understand that participation is voluntary and that completion of this registration and waiver is required for my child to be considered for BTG Basketball tryouts.

Scope of Waiver & Release of Liability

I hereby release, waive, and discharge BTG Basketball, Beyond The Game, its owners, directors, coaches, staff members, volunteers, affiliates, partners, event organizers, and facility operators from any and all liability, claims, demands, actions, or causes of action arising out of or related to my child’s participation in BTG Basketball tryouts.

This includes, but is not limited to, injuries, illness, property damage, or other losses that may occur before, during, or after tryouts, whether caused by negligence or otherwise.

Assumption of Risk

I understand that participation in basketball involves inherent risks, including physical contact, falls, collisions, overexertion, or injury. I knowingly and voluntarily assume all risks associated with my child’s participation, whether known or unknown, foreseeable or unforeseeable.

Behavior & Conduct

I understand that BTG Basketball maintains high standards for athlete behavior, sportsmanship, and respect. I agree that my child is expected to follow all instructions, rules, and codes of conduct set by BTG Basketball coaches and staff.

I acknowledge that failure to follow these standards, including unsafe or disruptive behavior, may result in removal from the tryout without refund.

Medical Consent & Athlete Readiness

I certify that my child is physically fit and medically able to participate in basketball activities. I agree to disclose any medical conditions, allergies, or limitations that may affect safe participation.

In the event of an emergency, I authorize BTG Basketball staff to seek medical treatment for my child if necessary. I understand that I am financially responsible for any medical care provided.

Photo & Video Release

I grant permission to BTG Basketball to photograph, record, and use images or videos of my child taken during tryouts for promotional, educational, and marketing purposes. This includes use on websites, social media, digital platforms, and printed materials, without compensation.

Acknowledgment of Understanding

I confirm that I have carefully read and fully understand this Participation & Waiver Release. I acknowledge that I am giving up certain legal rights on behalf of myself and my child and that I agree to these terms freely and voluntarily.
